Position Summary
Performs direct and indirect care in collaboration with team leaders including, but not limited to LPNs, RNs, Charge Nurse, Adult Day Program, Assisted Living and Home Health services.
Minimum Education Requirements
:High School diploma/GED or equivalent work experience in a relevant field (3 yrs +)
:Graduate of accredited certification course
Minimum Experience Requirements
:Previous experience working in a senior community preferred.
:CO Certified Nurse Aide (CNA)
:Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED.
Position Duties (Essential Functions)
Direct Care : 60
:Provides care as assigned in a manner which ensures resident/patient comfort and safety according to policies and procedures.
:Duties may include, but are not limited to assisting with Activities of Daily Living (ADLs), lifting, transferring, vital signs, weight, peri:care, dressing, reminders for meals, toileting, assisting with activities, appointments, etc. according to each facilitys guidelines.

:Reports status changes to charge nurse (or program leader) accurately and in a timely manner.
:Must be able to assist in orienting new CNAs and agency CNAs.
:Must keep current with information pertaining to the job by attending in:services, staff meetings, and special committees as assigned.
Informatics/Technology : 20
:Adapts to rapidly changing technology as evidenced by competency in technology and equipment used in the patient/resident care setting.
:Thorough and confidential maintenance of patient/resident charts and records.
:Document ongoing status, interventions, patient/resident response and outcomes in an accurate and timely manner.
:Use information technology to communicate, manage knowledge, mitigate error, and support decision:making.
Physical Requirements
:Medium Work : exert/lift up to 50 lbs force occasionally, and/or up to 20 lbs frequently, and/or up to 10 lbs constantly.
